two of my gyms have em....no kids for me, so I haven't used em, its a good question though...I want to adopt and will be needing that type of thing soon...always wondered if they were ok or not.

It's like anything else in life... you need to do research, attend a session to watch how the care givers interact, ask tough questions!!! You have a right to know and they shouldn't be offended that you want to protect your child. Ask them if they have ever had a case of abuse and how it was handled!!! ASK!!!
Even if you like the answers don't take it for granted that all is well, when you see new caregivers re-ask the questions, don't assume it was done, If you see alot of turnover, ASK.
Your child will be safer and get better care if the organization feels your involved and watching. They may talk behind your back, you may get a reputation as being difficult you may even feel they don't like you. SO F_IN WHAT. It's your child you are protecting.
Both my daughters went to some type of "care" and they may not remeber them but I'm sure they remember me......

my son loves it, they have a huge jungle gym with slides, bouncy house, toys, tv's video games, and afterwards he gets to chug on a nice chocolaty protein shake
They have camera's monitoring it from all angles, you can see them from any tv in the gym.
I for one don't mind if my son gets hurt playing, to me that is part of being a kid. It is when they can't explain how or why he is hurt that pisses me off. That goes for daycares, schools, anyone in general who happens to be in charge of care at that point.

I agree with A1, you can NEVER ask too many questions. Concerns to watch out for is hygiene and cleanliness......ask about things like how often do toys etc get disinfected. Be sure there is a sink in the area for handwashing and ask about toileting issues as well, if your child needs help in that dept. Best resource of all though is ask other members who use the service at your gym.....if most of the folks you talk to say they love it and thier kids never want to leave the playcare area.......you should be on it!!
